Transition to Adulthood 

The transition to adult services for young people with SEND is often fraught with anxiety, fear and challenge, not least because many services are still in the process of implementing the reforms of September 2014, and have not previously been ready tor provision to the new age limit of 25 years. 

However, there are a number of services well positoned to support, with information, advice and guidance and  practical tools to help with planning transition.

Preparing for Adulthood 
The Preparing for Adulthood (PfA) programme is funded by the Dept for Education as part of the delivery support for the SEN and disability reforms of September 2014. 
The partnership brings together a wide range of expertise and experience of working with young people and families, at a local and national level and across government, to support young people into adulthood with paid employment, good health, independent living options and friends, relationships and community inclusion.
